Scientists analyzed blood samples from 136 adults: 90 with Type 1 diabetes and 46 without. They measured five key proteins associated with neurodegeneration and brain inflammation: phosphorylated tau-181 (pTau-181), total tau, amyloid beta ratios, glial fibrillary acidic protein (GFAP), and neurofilament light chain (NfL).

After accounting for age, body weight, race, ethnicity, smoking status, and kidney function, the study found that people with Type 1 diabetes had significantly higher levels of three markers: pTau-181, GFAP, and NfL. All three are associated with damage to brain cells and neuroinflammation—the immune system's response in the brain.

What These Proteins Tell Us

Each of these biomarkers reflects a different aspect of brain health. Phosphorylated tau is a protein that builds up in neurodegenerative diseases like Alzheimer's. GFAP is released by support cells in the brain when they're stressed or damaged. Neurofilament light chain leaks into the bloodstream when nerve cells are injured.

Finding higher levels of all three in people with Type 1 diabetes suggests that the condition may affect the brain in multiple ways—through tau accumulation, glial cell stress, and actual nerve cell damage.

What We Don't Yet Know

This study is cross-sectional, meaning it captured a single snapshot in time. Researchers cannot determine whether these elevated biomarkers came before Type 1 diabetes developed, developed after diagnosis, or are simply associated with it. Longer-term studies would help clarify the timeline.

The study also doesn't explain whether controlling blood sugar or other diabetes management strategies reduce these biomarker levels or lower dementia risk. That remains an important open question for future research.

What This Means Going Forward

This research strengthens the connection between Type 1 diabetes and brain health, offering a biological explanation for the increased dementia risk observed in this community. Blood biomarkers like these could eventually become useful tools for identifying high-risk individuals.
